New Student Registration
Call the Attendance Office at 801-412-2905 for questions.
Please come between 8:30 to 11:00 or 1:00 to 2:00. The registration process may take up to one hour.
The following items are required by the Jordan School District and South Jordan Middle School before you may register for school:
- Birth Certificate1 – only if you are coming from a school outside of Jordan School District
- Proof of residency in South Jordan Middle School boundaries.2
- Print out and complete this Student Registration Packet.
- Official documentation of current Immunization Record
- Copy of I.E.P. (If student has ever received Special Education services)
- Withdrawal or Check-Out papers from the last school attended, with the school’s complete address and phone number.
- Transcript of past grades/report cards
- Check payable to South Jordan Middle School (cost may vary)

1 PARENTS/GUARDIANS: You must have legal guardianship of your student in order to register him or her in school. The Guardianship must come from a court of law, and be signed by a judge. If you do not have guardianship, please contact the Jordan School District Student Services Department at 801-567-8183.
2 Examples of a document that could be used as “Proof of Residency” are: water bill, power bill, or gas bill. PO Boxes are not accepted.
